For Your Visit

Thank you for making an appointment. Please download and complete our FORM 1 , FORM 2 and HIPAA consent .  Please complete all fields and answer all questions.  FORM 3  is your consent for us to obtain your medical records from other providers.

Please send us all lab work you had within last 12 months or bring it with you. Do not forget to bring all pertinent medical information from previous visits and consultations.

Please remember to bring forms to your appointment. THANK YOU!

We accept all Medicare plans. Our staff will provide you with all medical procedures and treatment options necessary for you to get better. It is your responsibility to check with secondary insurance if your visit is covered prior to seeing the doctor. You personal insurance plan may have certain limitations or restrictions for the treatments and procedures.


We offer discounted services to cash paying patients:

  1. If you were referred by our patient (you will need to mention the name)
  2. If you are a student or have low income
  3. Medical professionals
  4. Military veterans
  5. Special circumstances are always considered and approved by Doctor


We highly encourage each patient to check with Insurance carrier if we are IN or OUT of network. in general, we accept PPO plans from:

We offer exceptional services and treatments. Our cash prices reflect local medical charges. We are willing to make every effort to work out plan of payments for the diagnostic procedures and treatment.

  1. Cash price for your office visit and procedures will be presented to you in a treatment agreement and will be payable at the time of the visit. No hidden fees and multiple discounts available!
  2. You can use Flexible Spending Account to pay for all services provided in our office – we also accept debit cards or can provide you with a receipt and explanation of services for reimbursement. Check your eligibility for HSA or MSA in you have high-deductible plan or you would like to start SLIT treatment.
  3. If you do not have an insurance we take, you will be asked to pay for service at the time the service is provided. As a courtesy, we will generate and print an insurance claim form, which you will have to mail to your insurance company to receive reimbursement – the check will be sent to you directly from your insurance provider.

Please Be Considerate:

If you need to cancel your visit CALL AT LEAST 24 HOUR prior to your appointment. We always work hard to reschedule your visit at a time convenient for you. In order to maintain our schedule we will charge UP TO $50 FOR LATE CANCELLATION.
